Important Information about your Health Records

NHS England are now able to automatically extract personally identifiable information about you from our computer system, using its powers under the Health and Social Care Act 2012. At the end of January 2014 all households in England will receive an information leaflet called "Better information means better care". Please read the leaflet carefully. If you have not received the leaflet you can read it here.

If you do not want your details to be extracted, complete one of the opt out forms which can be obtained from Reception or downloaded from the Care Data page. Please note Patients who have opted out of the Summary Care Record (SCR) will still need to complete an opt out form.

 

 

 

Flu Vaccination Information

We are running Flu vaccination clinics in October for those patients who are most at risk. Please note we can only offer vaccines to patients eligible under the Department of Health's Flu vaccination program at this time.If you are eligible you will receive a letter in the post which you will need to bring with you to the clinic.

 

Eligible patients are:

 

All those aged 65 and Over

 

Patients living in long – term residential homes

 

Pregnant Women

 

Carers

 

Patients with the following conditions:

 

Chronic Lung Disease

 

Chronic Heart Disease

 

Chronic Renal Disease

 

Chronic Liver Disease

 

Chronic Neurological Disease

 

Diabetes

 

Immunosuppression or Immunodeficiency due to disease or treatment

 

 

 

If you do not have one of the above conditions and do not receive a letter, there are pharmacies, such as Boots and at Asda, that can provide these vaccinations for a fee .
