Important Information about your Health Records

NHS England are now able to automatically extract personally identifiable information about you from our computer system, using its powers under the Health and Social Care Act 2012. At the end of January 2014 all households in England will receive an information leaflet called "Better information means better care". Please read the leaflet carefully. If you have not received the leaflet you can read it here.

If you do not want your details to be extracted, complete one of the opt out forms which can be obtained from Reception or downloaded from the Care Data page. Please note Patients who have opted out of the Summary Care Record (SCR) will still need to complete an opt out form.

 

 

 

How to Register with the practice

To register you are required to fill out a registration form, a health questionniare and an Alcohol questionnaire. The government have also requested that we record your ethnicity and first language. All forms can also be found below. Please print them, fill them out and bring them in to the practice along with proof of identification as detailed below. PreRegistration

Proof of Identification

From 1st September 2011, following guidance received from NHS Bournemouth & Poole’s Counter Fraud Team, Patients wishing to register at the surgery, whether permanently or as a temporary resident, must now provide proof of Identification.

You will need to bring at least two of the following documents, one of which should contain your photo. If you wish to register permanently you should also bring at least one other document containing your home address.

  • birth certificate
  • marriage certificate
  • driving licence
  • passport
  • local authority rent card
  • paid utility bills
  • bank/building society cards/statements
  • National Insurance number card
  • payslip
  • letter from Benefits Agency/benefit book/signing on card
  • papers from the home office
  • P45.
