Important Information about your Health Records

NHS England are now able to automatically extract personally identifiable information about you from our computer system, using its powers under the Health and Social Care Act 2012. At the end of January 2014 all households in England will receive an information leaflet called "Better information means better care". Please read the leaflet carefully. If you have not received the leaflet you can read it here.

If you do not want your details to be extracted, complete one of the opt out forms which can be obtained from Reception or downloaded from the Care Data page. Please note Patients who have opted out of the Summary Care Record (SCR) will still need to complete an opt out form.

Practice Philosophy

elderly_consultThe aim of this practice is to meet the health needs of the people it serves.

We will, in order to achieve this:

  • Provide a service with appropriate care and consideration in accordance with our Practice Charter; the Dorset Health Authority and the National Patients Charters
  • Provide appropriate training to practice staff
  • Develop management systems which allow us to monitor the quality service delivered and identify new needs in the community
  • Communicate with our patients, colleagues and other agencies.

This charter has been published so that you know exactly what your rights are and the standards of healthcare you should receive.

A right is a level of service to which the patient is entitled and which must always be delivered.

A standard is a level of service which you can expect to be delivered other than in exceptional cases.

Please help us by letting us know if any of these rights or standards are not being delivered.
