Important Information about your Health Records

NHS England are now able to automatically extract personally identifiable information about you from our computer system, using its powers under the Health and Social Care Act 2012. At the end of January 2014 all households in England will receive an information leaflet called "Better information means better care". Please read the leaflet carefully. If you have not received the leaflet you can read it here.

If you do not want your details to be extracted, complete one of the opt out forms which can be obtained from Reception or downloaded from the Care Data page. Please note Patients who have opted out of the Summary Care Record (SCR) will still need to complete an opt out form.

 

 

 

Community Midwives

The Community Midwife is responsible for antenatal and early postnatal care.

She organises home visits and has a weekly antenatal clinic, she works in close co-operation with the doctors and the local maternity units.

She is available for advice on all aspects of maternity care, and for pre-conception counselling.

Community Nurses

There are three Community Nurses. They provide home nursing care and work closely with the Practice Doctors.

Health Visitors

She is concerned with the health of the whole community, especially the under 5's.

Attached Staff

Chiropody

This is available for the over 70's and medically at risk patients.

Private treatment is available at the Surgery for those who do not qualify under NHS Regulations.
